San Luis Valley Health has an opening for an Experienced Advanced Practice Provider in the Primary Care Department. A successful candidate must be a Board Certified or Board Eligible Nurse Practitioner or Physician Assistant and possess a desire and ability to work with a team in a collegial setting to provide comprehensive medical care.

Minimum Requirements:
  • Current state license as a Nurse Practitioner or Physician Assistant (credentialing requirement).
  • Eligible for State Controlled Substance License
  • Eligible for Federal DEA
  • BLS Certification
  • 3 years of experience required
