Full Job Description

About the Opportunity

A rural health system in northern New England has retained PX3 Medical to confidentially recruit its inaugural Director of Graduate Medical Education.

This is a foundational leadership role responsible for the launch, accreditation, and ongoing administration of a newly ACGME-accredited Family Medicine Residency Program welcoming its first class of PGY-1 residents in July 2027. It is a rare opportunity to build a residency from the ground up within a rural health system — one of the few new rural Family Medicine programs launched without an academic medical center partner.

Primary Responsibilities

  • Lead operational development and implementation of the inaugural Family Medicine Residency Program through July 2027 launch and beyond
  • Serve as institutional lead for ACGME accreditation, including the upcoming site visit and all subsequent reviews, annual reports, and documentation
  • Conduct a comprehensive gap analysis of current program readiness and drive completion of remaining infrastructure — curriculum, evaluation systems, onboarding, and policies
  • Manage the transition of GME oversight from the current FQHC sponsor to the health system
  • Establish and oversee the institutional GME office and supporting infrastructure
  • Coordinate residency recruitment strategy and NRMP/ERAS Match cycles
  • Manage GME budgets, grants, and CMS funding, including DGME and IME reimbursement
  • Manage Program Letters of Agreement with regional and out-of-state training partners
  • Partner with executive leadership on long-term GME strategy and future program expansion
  • Coordinate faculty development and support residency leadership, faculty, and clinical educators
  • Provide direct patient care in an outpatient family medicine clinic

Required Qualifications

  • Minimum 5 years of progressively responsible GME administrative experience (flexibility for candidates with a shorter timeframe and a demonstrated program-build track record)
  • Active MD/DO Physician License in Family Medicine or Med Peds.
  • Demonstrated experience supporting ACGME-accredited residency programs, including accreditation, compliance, and operations
  • Deep working knowledge of ACGME requirements, NRMP, ERAS, and GME best practices
  • Proven experience leading complex, multi-stakeholder projects
  • Willingness to be on-site to build direct relationships with providers and clinical partners

Preferred

  • Experience launching a new residency program and/or supporting initial ACGME accreditation
  • Rural health, critical access hospital, rural training track, community-based residency, or FQHC experience
  • Knowledge of CMS GME funding mechanisms and Medicare reimbursement (DGME/IME)
  • Experience establishing institutional GME infrastructure at organizations new to graduate medical education
  • Family Medicine residency program experience
  • Familiarity with Lean Six Sigma and high-reliability organization principles

Compensation & Benefits

  • Base salary:$350,000+, commensurate with experience
  • Incentives:Quality-based and milestone based plan to be co-designed with the CEO
  • Sign-on bonus and relocation: Both fully supported to meet market norms
  • PTO: 32 days year one, 35 days from year two.
  • Medical, Dental, Vision, HSA, FSA, DCA, Life, AD&D, STD, LTD
  • Retirement: 403b-1.5% match and 457b @ 15% of base, vest after 3 years

Why This Role

  • Ground-up build of an ACGME-accredited residency program, with full ownership of the program's institutional architecture
  • Directly-supported strategic mandate with dedicated state-level Rural Health Transformation funding
  • A CEO and executive team that hires for competence, provides resources, and stays out of the way
  • Clear runway for future program expansion beyond Family Medicine
  • Rural New England lifestyle

Northern New England's North Country offers a quality of life that's hard to match: four true seasons, immediate access to the White Mountains and the Connecticut River Valley, and a community pace that prioritizes rootedness over hustle. Outdoor recreation is at the doorstep — hiking, skiing, snowmobiling, fishing, kayaking, and the Appalachian Trail all within reach. Tight-knit communities, a low cost of living relative to the broader Northeast, and a region actively invested in the success of this program and the physicians who will train in it. This is a place where professionals put down roots — not a stop along the way.
