Major Responsibilities:
Provides care following the National Standard for Diabetes Self-Management Education and Support (DSMES) as defined by the American Diabetes Association (ADA) or Association of Diabetes Care and Education Specialist (ADCES).
Essential Functions
Conduct comprehensive assessments to identify patient learning needs, barriers to care, and readiness for behavior change.
Provide individualized diabetes self-management education and support for patients with gestational diabetes, Type 1 diabetes, Type 2 diabetes, and other pregnancy-related metabolic conditions.
Educate patients regarding nutrition, glucose monitoring, medication administration, physical activity, and healthy lifestyle behaviors.
Provide instruction and support related to insulin administration, CGM systems, and insulin pump therapy.
Review blood glucose logs, CGM reports, and insulin pump downloads to assess glycemic control.
Implement protocol-based treatment recommendations and insulin dose adjustments within established guidelines and scope of practice.
Collaborate with MFM physicians and APPs regarding patient progress and escalation of care.
Coordinate in-person, telehealth, and remote follow-up care.
Support postpartum diabetes screening and transition planning.
Participate in multidisciplinary care planning, quality improvement initiatives, and program development activities.
Licensure, Registration, and/or Certification Required:
Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred. If not certified, you must obtain certification within two years preferred but not to exceed 3 years. Minimum of two years of relevant clinical experience.
Education Required:
Registered Nurse (RN) licensure
Bachelor's degree in a qualifying healthcare-related discipline
Experience with high-risk obstetric populations, CGM systems, insulin pump therapy, telehealth, remote monitoring
Experience with Epic
Bilingual communication skills
Experience Required:
Minimum of two years of relevant clinical experience

Skills in public speaking (or potential to develop) for group/education classes or professional speaking opportunities
