Full Job Description
Job Summary:
Assists in the planning, development, and implementation of programs for the delivery of health services to patients served by the Health Centers. Attends at least 75% of quarterly medical staff meetings; attends any meeting as directed by medical director or his/her designee.

This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices' patient population. Participates in the practice's quality improvement process and performance evaluation.

Education:
Degree and completion of an approved residency program.

Licensure:
Licensed to practice in the State of Tennessee.

Certification:
Board certification in practice specialty.

Provides leadership in assessment, medical practice, and protocol development for the Health Centers.
Assists in strategic planning for the delivery of comprehensive health care services to the targeted population.
Develops formal relationships with community organizations, other health care providers, governmental agencies, and schools to assure the health status of patients.
Assures compliance with JCAHO/DNV, PCER, and other applicable regulatory agencies, guidelines, and regulations.
Prepares schedules for all medical staff.
Participates in the quality improvement program to ensure positive patient outcomes and maximum productivity.
Attends at least 75% of quarterly medical staff meetings; attends any meeting as directed by medical director or his/her designee.
