Full Job Description
Under the direction of the Administrator and Clinical Manager, the Clinical Nurse Liaison RN Hospice represents Yale New Haven Health System (YNHHS)/Yale New Haven Health - Health at Home in system hospitals and the clinical community by initiating and maintaining relationships with referral sources, patients and families and provides patient evaluations and patient care as needed.

Responsibilities

  • 1. Works with Administrator, Clinical Manager, and Manager of Referral Coordination to focus on agency and patient needs.
  • 2. Spends considerable time in YNHHS hospitals working directly with patients, care providers, and Care Management on care coordination and transition planning.
  • 3. Receive patient referrals electronically or via hard copy.
  • 4. Onsite evaluations of patients who may be eligible for Home Health and Hospice, coordinate with care providers, care management, and discharge teams in acute and rehab facilities to establish a care plan and/or safe transition to care.
  • 5. Perform on-site general inpatient (GIP) evaluations, admissions, and hospice GIP visits at assigned hospitals as needed.
  • 6. Tracks patients readmitted to hospitals.
  • 7. May deliver orders for physician signature.
  • 8. Meets with individuals and groups in the clinical community to educate regarding programs and services offered by Yale New Haven Health - Health at Home and Yale New Haven Health - Health at Home Southeast.
  • 9. Attends staff meetings, in-services, and all other required meetings.
  • 10. Compliance with all mandatory requirements inclusive of HealthStream.
  • 11. Assists with specialty projects and pilots such as remote patient monitoring, GIP, Advanced Care at Home and other as indicated.
  • 12. May perform other duties as assigned.

Qualifications

EDUCATION

Nursing diploma or an Associate degree in Nursing required. Bachelor's degree in nursing preferred.

EXPERIENCE

Minimum of four (4) years nursing experience in an acute care setting with at least one (1) year in Hospice required. Minimum of one (1) year Home Care and Hospice GIP experience preferred.

LICENSURE

Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within three (3) months (90 days) of hire.

SPECIAL SKILLS

Excellent verbal communication, customer service and written skills. Knowledge of home care and hospice eligibility desired. Must have self-motivation and excellent time management skills. Ability to effectively build relationships. Excellent computer skills.

PHYSICAL DEMAND

Periods of driving, walking, sitting, and standing. Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
