Responsibilities

Full Time Role

Starting Pay: 85,000 to 90,000 (Salaried role and pay is based on previous experience)

5 days per week schedule

On-call manager rotation

Position Purpose:

The Clinical Rehab Nurse Manager is an RN Nurse who serves in the capacity of acoordinator with assessing, planning, implementation, monitoring and evaluationof the options and services required to meet the residents' health serviceneeds.

Primary Job Duties:

  • Clinical Rehab Nurse Manager Participates in welcoming new residents and theirfamilies to the community
  • RN Initiates and supervises the completion of PhysicianCertification & recertification for rehab stay
  • Manages resident and family concerns in a calm &professional manner
  • Serves as a nursing representative for PAN, Bundle& Medicare meetings
  • Participates in resident baseline and comprehensivecare plan for continuity of care
  • Preparation, review and submission of insurancedocumentation, authorization, appeals and audits
  • Assessing, implementing, and evaluating residentindividual level of care needs and ensuring that is reflected in resident careplan
  • Monitors daily Nursing documentation to ensureresident condition and treatment is being appropriately documented
  • Participates in the Nurse on Call rotation
  • Other duties as assigned

Requirements:

  • Registered Nurse RN license in good standing
  • 3+ years clinical experience
  • Minimum 1 year experience in long term care
