Job Summary:The Internal Medicine Clinical Pharmacist Specialist (CPS) provides advanced, patient-centered pharmacotherapy management for adult internal medicine populations. Functions as an integral member of the interdisciplinary healthcare team to optimize medication therapy, improve clinical outcomes, and ensure safe, evidence-based use of medications.
Minimum Job Requirements:Work Experience:Three years of experience practicing in inpatient or health-system settings with direct patient care responsibilities or PGY1 Pharmacy Residency required. PGY2 Residency in Internal Medicine or related specialty preferred.
License/Registration/Certification:Current Texas Pharmacy License required
Board certification through the Board of Pharmacy Specialties (BCPS) or other related board certification such as BCGP, BCCP, BCCCP required.
Compounded Sterile Products Certified within 3 months of hire.
Current BLS and ACLS certification required.
Education and Training:Doctor of Pharmacy (PharmD) from an ACPE-accredited pharmacy program required.
Accreditation Council for Pharmacy Education (ACPE) approved continuing education per year to maintain licensure and BPS Certifications required.
Skills:- Knowledge of clinical pharmacology, pharmacokinetics, evidence-based medicine, and appropriate medication selection across diverse patient populations.
- Strong understanding of the full medication-use process, including selection, procurement, storage, prescribing, dispensing, administration, and monitoring.
- Knowledge of pharmacy policies, clinical guidelines, protocols, and interdisciplinary programs such as antimicrobial stewardship, opioid stewardship, anticoagulation management, and pharmacokinetic dosing.
- Knowledge of drug information resources, current evidence-based practice, and emerging therapeutic trends.
- Understanding of standards and sterile compounding regulatory requirements.
- Knowledge of federal, state, and local laws governing medication storage, handling, and dispensing.
- Skilled in providing clinical pharmacy consultation to healthcare providers, including therapeutic recommendations and interpretation of pharmacokinetic data.
- Ability to develop, implement, and maintain medication-use policies, procedures, and clinical protocols that support safe and cost-effective therapy.
- Strong communication and teaching skills for educating medical staff, pharmacy staff, patients, and families.
- Skilled in conducting quality improvement activities, medication-use evaluations, data collection, monitoring, and documentation.
- Proficiency in documenting clinical interventions and activities in the electronic medical record in accordance with regulatory and departmental standards.
- Skilled in precepting pharmacy residents and students, providing evaluation, mentorship, and professional development aligned with PGY1 residency standards.
- Ability to serve as a liaison between pharmacy, nursing, and medical staff to support medication-related policies, pathways, and best practices.
- Ability to participate effectively in interdisciplinary initiatives and clinical programs that optimize medication safety and therapeutic outcomes.
- Ability to maintain competency in sterile compounding, aseptic technique, and hazardous drug handling per organizational and regulatory requirements.
- Ability to interpret prescriptions for legal compliance and ensure safe, accurate medication dispensing.
- Ability to engage in clinical research activities, contribute to study protocols, and support data collection as required.
- Ability to function as a reliable drug information resource for healthcare providers, offering timely, evidence-based recommendations.
