Full Job Description
Job Summary

The Clinical Pharmacist would be responsible for providing pharmaceutical care and maintaining continuity of care for clientele through collaboration with Physicians, Nurses, other healthcare professionals and dispensary staff. The Pharmacist is expected to travel to different partner sites - such as medical clinics, group homes, Supportive Living and Long Term Care sites. At a minimum, the Pharmacist is expected to provide prescription adaptations, renewals, medication review services and support at the dispensary as needed. The Pharmacist is required to provide 24 hour emergency on call support in a rotation with other Clinical Pharmacists.

The Clinical Pharmacist practitioner would be responsible for maintaining continuity of care for clientele through collaboration between onsite Doctors, Nurses and dispensary staff at long term care and supportive living sites. They will provide prescription adaptations, renewals and three month review (TMR) services. They will be responsible to be relief coverage for Pharmacists that are on vacation to ensure continuity of care and well as provide Pharmacist support at our Fulfillment Centre. This position is required to provide 24 hour emergency on call support in a cycle with other Clinical Pharmacists.

  • To complete administrative and documentation requirements of clinical services within 24 hours of service provision
  • Maintain confidentiality and protect sensitive data at all times
  • Adhere to organization and department specific safety standards and guidelines
  • Work collaboratively with all colleagues and support the efforts of team members

Job Qualification
  • Licensed Pharmacist in Alberta
  • Strong preference for pharmacist with Additional Prescribing Authority
  • Strong preference for pharmacist with Authorization to Administer Drugs by Injection
  • Evidence of advancing one's own practice; additional certifications are an asset
  • Strong understanding of best practice guidelines and principles of pharmaceutical care
  • Able to provide evidence of advancing processes for clinical assessment and documentation
  • Experience in Continuing Care and/or multidisciplinary settings is an asset
  • Excellent computer knowledge
