  • Oversee the adverse drug reaction (ADR) program and provide drug information to medical staff

Job Summary and Qualifications

Clinical responsibilities:

1. Provide clinical pharmacy/pharmaceutical care for hospital patients;

2. Maintain current with clinical literature especially related to the hospital-specific patient populations;

3. Maintain competency to provide pharmaceutical care to all ages and types of patients that may be maintained within this hospital;

4. Serve as pharmacy representative for weekly team conference meetings;

5. Process automatic stop order forms;

6. Monitor patients on anticoagulant therapy, antibiotic therapy and make recommendations to physicians as needed;

7. Perform DUE as needed;

8. Serve as member of P&T committee and present reports as needed;

9. oversee ADR program;

10. provide drug information to hospital staff, administration and physicians.
Leadership and Supervisory responsibilites:

1. provide pharmacy students and residents guidance and education;

2. help provide annual goals for clinical program;

3. provide supervision of pharmacy technicians as needed
Operational responsibilities:

1. assume duties of staff pharmacist when needed which include:

a. Assumes duties of pharmacy technicians in their absence.

b. Observe and approve IV admixtures procedures and other preparations which technicians compound.

c. Supervise pharmacy technicians in the performance of their duties.

d. Attend Code Blues

e. Enters drug orders into the computer system without error.

f. Troubleshoot equipment/computer problems as needed

g. Perform quality monitoring functions as requested.

h. Obtain out-of-stock drugs or non-formulary drugs in accordance with policies in the absence of the purchasing agent.

I. Appropriately label and dispense medication in a timely manner.

j. Timely and complete follow-up of daily narcotic discrepancy report.

k. Check crash cart trays and code boxes in a timely manner

l. Approve and check re-packaged items.

m. Attend staff/hospital meetings as mandated.

n. Follow-up on any problem orders posted.

o. Dispense controlled substances in a timely manner and assist in maintaining appropriate inventory levels of controlled substances

p. Update and transmit TPN & chemotherapeutic orders to CAPS.

q. Timely and complete follow-up of daily MAR discrepancy reports.

r. Utilize pharmacist communication log daily.

s. Check e-mail daily (when scheduled) and respond in a timely manner.. ;

2. assist in interviewing, orienting and training pharmacy staff
Financial responsibilities:

1. help maintain the hospital formulary and drug budget;

2. help identify and monitor pharmaco-economic goals and plans;

3. monitor the use of high dollar medications and intervene as needed to provide effective and cost-appropriate outcomes;

4. provide monthly cost savings report directly relatedt o clinical pharmacy programs;

5. may be asked to attend medical staff meetings as a means of staying educated on important issues and to present proposals for new cost savings initiatives;

6. uses monthly staff meetings as a means of disseminating information regarding quality financial issues.
Customer Service Responsibilities:

a. Communicates to the nurse or practitioner any precautions necessary in the safe use of a drug;

b. Reacts positively to changes in the workplace ( to both increase or decrease the demands for services);

c. Encourages effective communication between pharmacy and other departments;

d. Provide printed drug information when requested;

e. Communicates efficiently with co-workers, gives and receives change of shift reports.
Performance Improvement Participation:

a. Is able to articulate departmental PI/QC activities.

b. Is able to articulate one of the Hospital-Wide PI activities.

c. Is able to state the methodology for PI used by the organization.

d. Participates in PI process by either assisting in data collection or serving as member of PI committee

Effective skills in regards to communication, presentation, leadership and teaching; proficient computer skills including Microsoft Word, Excel and Access.

Physical DemandsFrequently(at least 50%) - Standing/Walking. Usually(25-49%) - Sitting, Bending/Stooping, Using leg muscles frequently or for extended periods, Using back muscles frequently or for extended periods. Weight carried or lifted - 2-10 pounds and 11-20 pounds. Infrequently(less than 25%) - Turning, Lifting/Carrying, Pushing/Pulling, Climbing/ascending/descending ladders/stairs, Using arm muscles frequently or for extended periods, Carried or Lifted - 41-50 pounds and 51 pounds or more requires help. Working with or near chemicals, Working near radiation sources, Working with hazardous waste materials, Operating vehicles or machinery and Wearing protective clothing and/or equipment.

Working Conditions• Challenging work environment requiring good multi-tasking skills.
• Close interaction with customers: patients, physicians, administration and nursing & other departmental staff members
• May need to work more than 40 hrs a week and alternate shifts, when necessary
