Position Summary:

Clearway Health is seeking a Clinical Pharmacist! In this role you ensure optimal patient outcomes related to chronic and specialty drug-related disease states. Your tasks include preventing or resolving medication-related issues and optimizing drug therapy. You serve as a resource and drug expert to patients, families, physicians, nurses, and other healthcare professionals. Additionally, you play an integral role in educating pharmacists, residents, and students. As needed, you may also be involved in coordinating the acquisition and distribution of drug therapy.

Essential Responsibilities/Duties:
  • Conduct comprehensive medication therapy reviews for patients on specialty medications, ensuring safety, efficacy, and adherence.
  • Provide disease-specific counseling and education to enhance patient understanding and engagement in therapy.
  • Partner with prescribers, nurses, case managers, and payors to facilitate initiation and continuity of specialty therapies.
  • Support prior authorization, appeals, and benefits verification processes to ensure timely patient access to treatment.
  • Monitor clinical outcomes, adverse events, and adherence metrics to evaluate therapy effectiveness.
  • Document and report interventions and outcomes to support quality improvement and compliance.
  • Ensure all specialty pharmacy activities adhere to URAC/ACHC standards, manufacturer agreements, and payor requirements.
  • Maintain documentation and reporting to support audits and accreditation readiness.
  • Participate in developing clinical protocols, therapeutic management programs, and formulary strategies.
  • Lead or support initiatives that optimize workflow efficiency, patient satisfaction, and overall program growth.


Education:
  • A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
  • Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
  • Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy Residency/Fellowship) is preferred.


Certificates, Licenses, or Registrations:
  • An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
  • Must possess or be able to obtain a State Board of Pharmacy Preceptor's license by the current state you are applying to within one year of hire.


Knowledge & Skills:
  • Demonstrated expertise in medications, drug interactions, medical procedures,
  • therapies, diagnoses, and disease states is essential.
  • Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues.
  • Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines.
  • Organized work approach with a proficiency in maintaining accurate documentation.
  • Strong verbal and written communication skills; proficiency in additional languages is advantageous.
  • Self-directed with strong self-management abilities; capable of identifying and
    addressing issues independently.
  • Proven track record in process improvement and a keen eye for detail and precision.
  • Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.


Special Working Conditions (On-call, Travel, Shift, Coverage):
  • Clearway Health Specialty Pharmacies, serving patients in hospitals, health systems, and communities, may require employees at client sites to work on company-recognized holidays.


This information is being provided to promote pay transparency and equal employment opportunities at Clearway Health. The current annual compensation range for this position is $133,000 - $166,000. The actual rate within this range that you will be offered will depend on a variety of factors including geography, skills and abilities, education, experience and other relevant factors. In addition to a competitive base salary, this role is eligible to participate in the company's bonus program as part of its comprehensive and rewarding benefits package.
