Status: Full-Time, W2 Employee
Schedule: Monday - Friday, 8 AM - 5 PM (Occasional on-call weekends)
Location: Arizona (West Valley, East Valley, and Tucson)
About the RoleAs a Clinical Patient Care Navigator, you will have the unique opportunity to utilize your clinical background to connect with patients, families, and referral partners with our exceptional continuum of healthcare services. In this role, you will focus on strategically expanding our business within your assigned territory while strengthening existing partnerships and fostering new connections. You will play a key role in identifying and generating opportunities to grow our services for Atlas Medical, Aegis Home Health, and Aegis Hospice. Your responsibilities will include representing our organization, building meaningful relationships, and delivering impactful presentations on Home Health and Hospice care. Additionally, you will collaborate with our team to achieve monthly goals and ensure the highest standards of service and care. QualificationsTo thrive in the role of Patient Care Navigator, you must:
- Have Hospice Clinical background
- 2+ years of Hospice Referral Relationships
- Knowledge of local Senior Care Industry, Hospital Case Management, and Skilled Nursing
- Intuitive, solution-oriented, and driven to achieve goals
- Detail-oriented, flexible, and self-motivated
- Excellent oral and written communication skills
- Valid Driver's License (clean record) & reliable transportation
- Proof of current auto insurance & car registration
- Valid AZ Fingerprint Clearance Card (we can help with this)
- Current CPR Certification (we can help with this) & Negative TB Test (we can test)
