RN Clinical Manager Home Health ServicesKey Responsibilities:- Provide daily clinical oversight of patient care services.
- Coordinate referrals and admissions with internal and external sources.
- Evaluate patient needs and agency capabilities before admission.
- Supervise care planning, partner assignments, and care delivery.
- Ensure compliance with regulatory standards and agency policies.
- Mentor and coach clinical staff to promote morale and retention.
- Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Qualifications:Required:- Active, unrestricted RN license in the state of practice.
- Minimum of two years RN experience (preferred)
- At least one year of Home Health experience (preferred)
- Current CPR certification.
