Full Job Description
Responsible for maintaining the quality and development of clinical mental health services, assessing and meeting the community health needs and optimizing the practice of nursing care delivery to the community.

Responsibilities

  • 1. Performs all duties in compliance with all applicable federal, state and local laws and regulations, as well as with the YNHHS code of ethics and policies and procedures at all times. Regular attendance is an essential job function of this position.
  • 2. Participates in development of agency objectives, standards of care, and clinical policies and procedures for the department.
  • 3. In conjunction with the Assistant Psych Manager, supervises psychiatric home nursing services and evaluates patient care provided by staff under his/her direction to ensure standards of practice are maintained.
  • 4. Oversees new patient assignments and ongoing schedules. Collaborates with Assistant Manager and staff on ensuring appropriateness of clients for home care.
  • 5. Directly evaluates the clinical competence of nursing personnel.
  • 6. Oversees the appropriateness of clinical documentation to ensure it meets all requirements.
  • 7. Facilitates case management and coordination of services.
  • 8. Incorporates Agency strategic initiatives within the department.
  • 9. Promotes learning and professional growth of staff by providing orientation and ongoing feedback, ensuring availability of educational programs, and performing annual competency evaluations.
  • 10. Responsible for maintaining appropriate staffing levels and for recruitment and staff selection.
  • 11. Collaborates with the sales team and marketing department to promote new referrals and participate in sales calls.
  • 12. Manages department budget in line with YNHHS and department goals.
  • 13. Collaborates with system finance staff regarding patient insurance eligibility.
  • 14. Works with patients and other mental health support agencies regarding patient's needs, safety and insurance issues.
  • 15. Promotes the mental health program to internal and external customers
  • 16. Collaborates with other departments to promote smooth processes.
  • 17. Performs other job duties as required

Qualifications

EXPERIENCE

An agency supervisor of clinical services shall be a registered nurse with an active license to practice nursing in this state, and shall have one of the following:
(1) A master's degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one year (1) fulltime clinical experience in a home health agency or related community health program which included care of the sick at home; or
(2) A baccalaureate degree in nursing and a minimum of three (3) years of full-time clinical experience in nursing, at least (1) one of which was in a home health agency or community health program which included care of the sick at home; or
(3) A registered nurse who has been continuously employed in the position of supervisor of clinical services in a home health agency in this state since January 1, 1979; or
(4) A diploma in nursing or an associate's degree in nursing and
(A) A minimum of three years of full-time or full-time equivalent clinical experience in nursing within the past five years, at least one year of which was in a home health care agency or community health program which included care of the sick at home; and
(B) Evidence of certification by the American Nurses Association as a community health nurse or completion of at least six credits received within two years in community health nursing theory or six credits in health care management from an accredited college or university program or school of nursing.

PHYSICAL DEMAND

The physical demands of this position range from sedentary to light; i.e., constant handling or carrying of 10 pounds or less, walking and carrying at self pace. Handling is performed on a constant basis. Sitting, standing, walking and bending are performed on a frequent basis and carrying is performed on a regular basis. The highest physical demands in this position include the moving, lifting and carrying of files, charts and other documentation, all of which involve handling loads up to 10 pounds on a regular basis; and carrying or moving loads up to 20 pounds are performed on an occasional basis.
