  • Conduct timely patient assessments as needed

  • Identify trends requiring intervention based on department performance

This position is incentive eligible.

Job Summary and Qualifications

May perform patient assessments as needed.

  • Assures patient assessments are completed in a timely manner by the department
  • Provides direct clinical oversight of the patient access process
  • Works collaboratively with hospital departments to provide services needed.
  • Models and trains other behavioral health team members on managing and tracking inquiry calls.
  • Tracks and reports on all key indicators related to calls, assessment, admissions.
  • Ensures patient rights and confidentiality are maintained
  • Provides input, suggestions and support appropriately.
  • Participates as an active member of the multidisciplinary team.
  • Responsible for reporting department performance and for identifying trends/patterns that require intervention
  • Completes additional duties as assigned.


What qualifications you will need:
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
  • Masters Degree
  • (LCSW) Clinical Social Worker, or (LPC/LMHC) Licensed Professional/Mental Health Counselor, or (LMFT) Licensed Marriage and Family Therapist, or (LMSW/RCSW-I) Lic Master Social Worker
