JOB STATEMENT/SUMMARYThe Clinical Care Director leads day-to-day nursing and certified medical assistant (CMA) operations across the health center's primary care and pediatric sites. The director supports consistent, safe workflows; coaches and deploys staff; and works with medical, operations, quality, and population health leaders to improve access, continuity, patient experience, and outcomes. The role supports care models and payer arrangements that reward preventive care, chronic disease management, coordinated care, and measurable quality improvement. The director does not replace the health center's designated medical leadership.
CORE RESPONSIBILTIESClinical Team Leadership and Daily Operations• Direct, coach, and evaluate nurses and CMAs through site leads or directly, as assigned; clarify responsibilities, set expectations, address performance concerns, and support retention and professional development.
• Coordinate staffing plans, schedules, cross coverage, orientation, and competencies across locations in response to patient volume, acuity, pediatric needs, and access targets.
• Conduct regular site rounds and team huddles; remove barriers in rooming, triage, patient related calls, patient care related fax and prior authorization work as applicable, standing orders, specimen handling, medication and vaccine workflows, and timely escalation.
• Partner with site management and clinical management leaders to standardize workflows while adapting to local patient needs; support consistent documentation and handoffs in the electronic health record.
Clinical Safety and FQHC Readiness• Maintain, participate and train staff on approved clinical procedures, infection prevention, emergency response, medication and vaccine storage and administration, laboratory processes, and pediatric safeguards; investigate gaps and track corrective actions.
• Ensure nursing and CMA assignments, delegation, standing orders, and supervision follow applicable state law, individual credentials and competencies, and health center policy; escalate scope or clinical judgment questions to the appropriate licensed clinician.
• Partner with credentialing, quality, and human resources teams to keep required clinical staff records, competencies, certifications, and applicable privileges current; prepare teams for audits and Health Center Program reviews.
• Participate in the board approved quality improvement and assurance program, patient safety event review, grievance follow up, and periodic assessments; protects patient information and promotes a just reporting culture.
• Participates in development, revision and updating of clinical policies, procedures, and
protocols to assure best practices in healthcare.
• Collaborates with management, performing regular, periodic onsite site visits ensuring
nursing and certified medical assistant employee performance is compliant with policy,
protocols, and clinical best practices in patient care. Interacts with clinical staff, monitoring
and ensuring accurate performance of diagnostic labs, medication administration, patient
care procedures, clinical documentation, and quality control management of in-house
diagnostic testing equipment.
• Oversees hazardous waste management and compliance in clinical operations.
Primary Care, Pediatrics, and Value Based Care• Support and assist in coordination of team workflows for well child visits, immunizations, developmental and other age appropriate screenings, adult preventive services, chronic condition follow up, and patient education under approved clinical guidance.
• Ensure completion of nursing and CMA patient care responsibilities in outreach, care gap closure, referral tracking, transitions after emergency department or hospital care, and follow up for patients with complex needs with care managers, community health workers, behavioral health, and providers.
Collaboration and AccountabilityWorks closely with the Chief Quality Officer, clinical management, site operations, quality improvement, population health, pharmacy, laboratory, infection prevention, radiology, HR, and finance. Recommends staffing and contributes to budgets and service planning, and reports operational and quality risks with proposed actions. Participates in assigned committees and communicates decisions consistently across sites.
Performs other duties as assigned by the CQO.
MINIMUM QUALIFICATIONS• Current, unrestricted registered nurse license in the state or states of practice; bachelor's degree in nursing preferred.
• At least five years of ambulatory or primary care nursing experience, including at least three years leading clinical teams; experience with pediatric care and multiple sites strongly preferred.
• Working knowledge of RN and CMA scope, delegation, immunization and medication safety, ambulatory triage, quality improvement, EHR workflows, and population health reporting.
• Ability to lead across disciplines, use data to improve work, address performance issues fairly, and communicate with patients and staff from varied backgrounds. Current BLS and ACLS certification and other credentials required by the organization.

MEASURES OF SUCCESSPerformance is evaluated against organization approved goals, such as safe staffing and competency completion; timely triage and follow up; vaccine and medication process reliability; patient and staff experience; preventive and chronic care quality measures; care gap and referral closure; and documented improvement across sites. Targets and measures are set annually with medical and quality leadership.
WORK REQUIREMENTSRegular travel among health center locations is required. The role may respond to urgent clinical operations needs outside normal hours. Essential functions include onsite observation, staff coaching, EHR use, and participation in clinical safety response. Reasonable accommodations are available in accordance with applicable law.
