No weekends, No holiday, No on call Sign on Bonus: $15,000Position Summary: The Wound, Ostomy, and Continence Nurse (WOCN) provides advanced clinical expertise, consultation, and education related to wound, ostomy, and continence care for patients receiving services from University of Rochester Medicine Home Healthcare. This role supports high-quality, evidence-based care delivery, regulatory compliance, and clinician education while partnering closely with interdisciplinary team members to optimize patient outcomes in the home setting.
The WOCN functions as a clinical expert, educator, and resource for complex wound and ostomy management, supporting both direct patient care and agency-wide quality initiatives.
Minimum Qualifications: - Bachelor's Degree in Nursing required, Master's Preferred.
- WOCN Certification and experience required.
- 2-3 years of clinical nursing experience, with demonstrated expertise in wound and ostomy care, preferably in the home care setting.
- New York State licensed RN required
Desired Qualifications: - Advanced knowledge of wound healing principles and evidence-based wound care practices. Strong assessment, critical thinking, and clinical decision-making skills.
- Ability to educate and support clinicians, patients, and caregivers with varying levels of health literacy.
- Effective written and verbal communication skills
Responsibilities - Provides expert assessment, planning, implementation, and evaluation of wound, ostomy, and continence care for patients in accordance with provider orders, CMS Conditions of Participation, and New York State Department of Health regulations.
- Performs comprehensive wound assessments, including etiology, staging, measurement, risk factors, and develops individualized care plans via home visits and or co-visits. Recommends appropriate treatment modalities, dressings, and supplies based on evidence-based practice and patient-specific needs.
- Completes a minimum of 5-10 home visits and/or co-visits weekly.
- Supports management of complex wounds, ostomies, fistulas, pressure injuries, diabetic foot ulcers, surgical wounds, and incontinence-related skin conditions.
- Serves as a clinical consultant to field clinicians, case managers, and providers regarding wound and ostomy care concerns.
- Educates patients, families, and caregivers on wound, ostomy, and continence management in the home setting. Provides education, mentoring, and clinical support to home health clinicians to enhance competency and confidence in wound and ostomy care. Participates in onboarding, orientation, and ongoing education related to wound care best practices. Supports competency validation and skill development for clinical staff as assigned.
- Collaborates with Quality, Education, and Clinical Leadership to support regulatory compliance, documentation accuracy, and quality improvement initiatives.
Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.
Salary Range - $76,606 - $107,993The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
