About the PositionPlease note: The salary range above represents this position's earning potential. The anticipated hiring range for this position will be $64,104 to $91,003, based on the candidate's qualifications and experience.Aging and Disability Home Care program is seeking a dynamic, dedicated and professional Certified Nursing Assistant to provide hands-on, personal care and personal hygiene assistance to disabled adults, frail elderly, mentally, chronically, and terminally ill and intellectually disabled adults, aged 18 and over.
These clients are seen in the community in their homes. The employee will need to be able to facilitate a safety plan with clients in crisis situations. The employee should be able to work well in a multi-disciplinary team. The employee in the position must have reliable transportation and be able to work well independently. The Home Care team provides care to complex clients in a highly therapeutic and supportive manner, helping vulnerable individuals maintain independence in the least restrictive setting. The employee will be part of a supportive and supervised setting, gaining a high level of support from supervisors and co-workers.
What You'll Be DoingThis position is a supportive service for the Adult Protective Services (APS) and Social Services To Adults (SSTA) programs, providing hands on personal care assistance, as well as supportive therapeutic services. Goals of the program are promotion of a safe environment, providing long term maintenance care, and to promote independence, activity, proper diet, and social contact.
Duties include:• Provision of hands-on assistance with personal hygiene
• Follows a case manager-driven goal/care plan that includes, but is not limited to, therapeutic services, personal care, light housekeeping, laundry, and transportation
• Helps clients set and reach realistic goals for improving ability to safely care for self, children, or dependent adults
• Collaborate with case managers to develop, monitor, and modify clients' treatment plans
• Provides positive and appropriate role modeling behaviors
• Provides and assists clients with information about available resources and assists them in making use of these supports that may help change conditions or behavior.
• Works with clients, caretakers, and social workers to help clients overcome obstacles related to emergency preparedness
This is a position that will work in the community. The selected candidate will travel throughout the County to perform job duties and transport necessary supplies and equipment. Position will require prolong standing, walking, and lifting up to 35 pounds.
Limited telework may be an option at times to complete administrative tasks, but it is NOT a guarantee that it will continue or be available indefinitely due to changes that may occur in County operations, services, or needs.
The selected candidate will be required to successfully complete a pre-employment background check, medical exam and drug screening.
Minimum QualificationsExperience: Three (3) years of experience providing delivery of social, health and/or public assistance services.
Education: Completion of high school or High School Certificate of completion recognized in the State of Maryland
Equivalency: An equivalent combination of education and experience may be substituted.
Driver's License Required: Possession and maintenance at all times of a valid State of Maryland driver's license is required.
Licensure: Possession and maintenance at all times of a current Maryland Certified Nursing Assistant (CNA) certification.

• Customer service experience working with Older Adults and people with disabilities with knowledge of family dynamics, human development, and crisis intervention through a crisis service such as adult protective services; ability to establish and maintain effective working relationships with clients of a wide variety of socioeconomic levels and cultural backgrounds.
• Experience communicating information clearly and effectively preparing and documenting client notes in addition to statistical summary reports on paper or electronically.
• Experience working in multi-disciplinary teams such as nurse, social workers, and experts from other health related fields.
• Bilingual language proficiency
