CBI Home Health is looking to hire a Care Manager - Therapy Services Hamilton

Salary Range: The salary range for this position is $80,000 - $90,000 annually.

What you will need to be successful

  • Have a master's or bachelor's Degree in a Regulated Rehabilitation Health profession
  • Possess a current registration with the applicable regulatory College in the province of Ontario
  • 3 to 5 years of leadership, supervisory, or home health care experience.
  • Current clinical knowledge and a commitment to delivering high-quality patient care.
  • Strong leadership, coaching, team development, and employee engagement skills.
  • Excellent interpersonal, communication, conflict resolution, and customer service abilities.
  • Proven analytical, critical thinking, problem-solving, and decision-making skills.
  • Experience with project management, change management, quality improvement, and risk management.
  • Financial acumen, including budgeting, forecasting, and performance management.
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with Microsoft Office and healthcare scheduling systems.
  • Valid driver's licence and access to a reliable vehicle.

  • Manage risk, support audits and investigations, and ensure compliance with regulatory and organizational standards.
  • Support financial and workforce planning to maintain appropriate staffing levels, service excellence, and sustainable program performance.
