Full Job Description
Care Connections is searching for a full-time Nurse Practitioner OR Physician Assistant to join our team!

Care Connections is a unique, temporary program for patients with chronic medical conditions who require more time and attention. Our goal is to bridge the gap between higher acuity inpatient services and the patients primary care office. Our providers see patients in both inpatient, outpatient, and home care settings. Our team includes social workers, nurses, rehabilitation therapists, pharmacists, and other health care providers. Additionally, patients in the Care Connections program have longer appointments, allowing for more time with the provider. Our providers enjoy a Monday through Friday schedule, dayshift only, with minimal call. Our team-based holistic approach to patient care allows us to focus on patient well-being and provider satisfaction.

The ideal candidate for this role will be interested in managing complex conditions, experienced with vulnerable populations, and a team player. New graduate NPs and PAs will be considered for this role with appropriate undergraduate experience.

Minimum Qualifications:
  • Current Licensure as a Physician Assistant OR Nurse Practitioner
  • Valid, unrestricted Federal Drug Enforcement Agency (DEA) number
  • Participation as a provider in good standing in the Medicare, Medicaid, and Tricare programs and such other third-party-payment programs, including managed care programs in which PMMG participates.
