Advanced Practice Provider - Advanced Lung Disease- COPD

Qualifications

• Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) certification required.
• Experience in outpatient management of chronic respiratory conditions, particularly COPD.
• Strong skills in patient education and communication.
• Ability to coordinate care with multidisciplinary teams.
• Familiarity with medication and inhaler optimization strategies.

Responsibilities

• Manage longitudinal outpatient care for patients with COPD.
• Conduct new and follow-up visits to assess patient progress.
• Optimize medication and inhaler use for effective treatment.
• Oversee oxygen management and adjust as necessary.
• Review diagnostic tests and integrate findings into care plans.
• Educate patients on COPD management and self-care strategies.
• Coordinate with pulmonary rehabilitation and other services to enhance patient outcomes.
