Full Job Description

Summary of Essential Responsibilities

The APP will work closely with a physiatrist (a physician specializing in physical medicine and rehabilitation) to:

  • Perform acute care consultations for hospitalized patients.

  • Evaluate patients to determine the most appropriate level of post-acute care, such as: Inpatient rehabilitation, Skilled nursing facility, Home with therapy services, other rehabilitation settings

  • Assist the acute care rehabilitation specialist with inpatient rehabilitation consultations.

  • Collaborate with Mary Free Bed Rehabilitation Hospital physiatrists and the multidisciplinary care team to coordinate patient care.

  • Work primarily at Mary Free Bed Hospital while providing consultation services for patients admitted to Corewell Health's acute care hospital in Grand Rapids, Michigan.

Schedule

  • Full-time

  • Monday through Friday

  • Weekend rotation of approximately 7-8 weekends per year

What this role requires

This position is well suited for a Nurse Practitioner (NP) or Physician Assistant (PA) with experience in:

  • Hospital medicine

  • Physical Medicine & Rehabilitation (PM&R)

  • Neurology

  • Trauma or orthopedic services

The work is generally consultative rather than procedural, with a focus on:

  • Comprehensive patient assessments

  • Functional evaluations

  • Discharge planning

  • Determining rehabilitation candidacy

  • Coordinating care with physicians, therapists, case managers, and nursing staff

This is a collaborative inpatient role centered on helping hospitalized patients transition to the rehabilitation setting that best matches their medical and functional needs.

Customer Service Responsibilities  

Demonstrate excellent customer service and standards ofbehaviorsas well as encourages, coaches, and monitorsthe same inteam members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.

Responsibilities in Quality Improvement  

Contribute byidentifyingproblems and seeking solutions. Promote patient/family satisfaction where possible; participatesin departmental efforts to monitorand report customer service.

Essential Job Qualifications  

  • Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan.

  • Master’s Degree in Nursing or Physician Assistant.

  • 3-5 years of experience with neurologically impaired patients

  • Current Basic Life Support (BLS) certification
