Qualifications

• Licensed as a NP or PA in the relevant state
• Federal DEA License/eligibility required
• Preferred experience in geriatrics
• Active CPR/BLS Certification
• Board Certification required
• Valid driver's license with a clean record

Responsibilities

• Manage chronic conditions for a high-risk Medicare/Medicaid geriatric patient panel
• Focus on quality patient care over patient volume
• Collaborate with a team of 20+ providers
• Utilize a patient-centered approach to address individual needs
• Engage in continuous learning and improvement in geriatric care
