Advance Practice Provider (NP/PA) - Value-based Primary Care

Qualifications

• Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the relevant state.
• Possess a Federal DEA License or eligibility.
• Preferred experience in geriatrics and home health.
• Current CPR/BLS Certification required.
• Board Certification is mandatory.
• Valid driver's license with a clean driving record.

Responsibilities

• Provide patient-centered care focusing on quality over volume.
• Conduct thorough assessments during 30-45 minute patient visits.
• Manage a patient panel across multiple locations including Denton and Carrollton.
• Collaborate with interdisciplinary teams to enhance patient outcomes.
• Educate patients and families on health management and preventive care.
