Qualifications

• Licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in the relevant state.
• Possession of a Federal DEA License or eligibility.
• Preferred experience or training in geriatrics.
• Current CPR/BLS Certification.
• Board Certification required.
• Valid driver's license with a clean driving record.

Responsibilities

• Manage chronic conditions for a high-risk geriatric patient panel.
• Provide patient-centered care with a focus on quality over volume.
• Utilize Athena EMR and Marvix AI Scribe for efficient charting during work hours.
• Collaborate with interdisciplinary teams to enhance patient outcomes.
• Conduct thorough assessments and develop individualized care plans.
