Responsibilities
JOB SUMMARY: To provide information about Hospice Home Care programs to patients, families, physicians, discharge planners and community organizations, and to admit patients to the appropriate programs.
ESSENTIAL FUNCTIONS:
• Main point of contact to answer questions about home hospice services to prospective patients/families
• Thoroughly explain insurance benefits and coverage of services, patient responsibilities and options for alternative means of payment.
• Coordinate the admissions process to the appropriate program and complete paperwork promptly and accurately
• Develop the initial plan of care based on input from patient, family and physician.
• Provides information and education to physicians, discharge planners and community organizations about Stella Maris Home Hospice services.
• Assumes on-call responsibilities as needed
Requirements
• RN licensure in the State of Maryland required
• EPIC experience preferred
• Prior experience in hospice or home health required
• Valid driver's license and reliable transportation

Salary Information
