What You’ll Do:
- Complete an initial, comprehensive and ongoing assessments to determine home health and/or hospice needs.
- Develop patient focused care plans that establish goals, based on nursing diagnosis and incorporates palliative nursing actions.
- Provide respectful, compassionate care in patients27 homes.
- Act as Case Manager and assume responsibility to coordinate patient care for an assigned caseload.
- Collaborate with interdisciplinary team members.
- Communicates with the physician regarding the patient27s needs and reports changes in the patient27s condition; obtains/receives physicians27 orders as required.
What You Bring:
- Active RN licensure (state-licensed).
- Valid driver27s license, reliable transportation, and current auto insurance.
- Strong communication skills and a team-oriented mindset

Wage: $38 - $40 per hour
