Title: Nurse Practitioner - Indigenous Health Promotion and Disease Prevention

Status: Permanent Full-time
Eligible for WAHA Sign-on Bonus (up to $15,000), Employee Referral Bonus (up to $5,500) &Retention Program Who You Are - Knowledge of health promotion, disease prevention, and population health principles, including use of data and quality improvement approaches to inform care
- Understanding of Indigenous determinants of health, health inequities, and the application of culturally safe, trauma-informed, and Indigenous harm reduction approaches
- Understanding of traditional and land-based approaches to wellness and the unique context of the James and Hudson Bay region
- Strong clinical assessment, decision-making, and problem-solving skills within NP scope of practice
- Ability to collaborate effectively across interdisciplinary teams and programs without supervisory responsibility
- Demonstrates respect for and alignment with Indigenous perspectives, values, and ways of knowing, applying culturally safe and trauma-informed approaches
What You'll Do - Delivers and supports culturally relevant health promotion and wellness initiatives grounded in Indigenous perspectives of health and well-being
- Provides education on preventative health topics, including nutrition, physical activity, tobacco cessation, sexual health, mental wellness, and chronic disease prevention
- Engages with communities to increase awareness and participation in health promotion and prevention initiatives
- Supports community-informed approaches by working in partnership with Elders, knowledge keepers, and local leadership
- Works collaboratively with WAHA programs including Diabetes Education, Chronic Disease Prevention, Lower Limb Preservation, Indigenous Harm Reduction, Mental Health and Addictions, Community Paramedicine, Dental and others to support prevention and early intervention pathways
- Leads a coordinated regional smoking cessation initiative in collaboration with internal programs, external partners, and the communities we serve to support prevention and screening across the continuum of care
- Completes focused assessments related to prevention, screening and early identification of health risks
- Initiates appropriate referrals and consults to support timely access to care
- Provides short-term NP primary care coverage across WAHA sites when operationally required
- Delivers both virtual and onsite primary care during coverage periods
- Travel is required to the communities we serve from time to time for NP coverage and for Health promotional activities and Community engagement
What You Bring - Successful completion of a Registered Nursing Program with certification as a Primary Health Care Nurse Practitioner.
- Registered with CNO as a Registered Nurse Extended Class (RN EC) and in good standing
- BCLS-HCP, required
- ACLS, PALS, TNCC (or willingness to complete)
- Added certifications (acute, geriatric, mental health, women's health) considered an asset
- Minimum of 3-5 years recent experience as a Nurse Practitioner in a primary care, community, or ambulatory care setting
- Ability to communicate in the local native Cree language is an asset

Salary: $159,588.00 - $165,886.50
