Facility: St. Vincent PACE

Department: Homecare

Schedule: Full-Time, Weekends as needed

Salary Range: $35.46 - $50.05 per hour

How you'll make an impact in this role

  • Delivered skilled home-based nursing care to PACE participants, including assessments, acute triage, wound care, blood draws, medication management, and chronic disease monitoring.
  • Conducted in-home visits and coordinated care across healthcare providers, assisted living, and long-term care settings to support aging in place and reduce hospitalizations.
  • Collaborated with the Interdisciplinary Team (IDT) to develop and implement participant-centered care plans that addressed medical, functional, and psychosocial needs.
  • Educated participants and caregivers on disease management, medication adherence, safety, and end-of-life care while providing after-hours and on-call nursing support.

What minimum requirements you'll need

Licensure / Certification / Registration:

Required Credential(s):

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Registered Nurse obtained prior to hire date or job transfer date.


Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional preferences we're seeking

Long term care experience highly preferred.
