What You’ll DoAs a Home Health RN, you will:
Manage an assigned caseload of home health patients
Complete comprehensive assessments and ongoing evaluations
Develop, implement, and update individualized plans of care
Coordinate services with physicians, therapists, caregivers, and clinical leadership
Educate patients and families on medications, treatments, disease management, and safety
Monitor changes in each patient’s condition and communicate significant findings
Maintain accurate, timely clinical documentation
Help patients achieve better outcomes while remaining safely at home
What You’ll NeedCurrent, unrestricted RN license
At least one year of clinical nursing experience
Associate degree in Nursing required; bachelor’s degree preferred
Previous home health or hospice experience preferred
Homecare Homebase experience preferred
Dependable vehicle, valid driver’s license, and current auto insurance
Ability and willingness to travel throughout the assigned territory
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
