The Inpatient Care Manager is responsible for coordinating patient care across the continuum of illness to ensure high-quality outcomes and efficient use of healthcare resources. Working closely with physicians, nurses, and interdisciplinary teams, this role leads care planning, utilization review, and discharge coordination while proactively addressing barriers to care. By combining clinical expertise with collaboration and oversight, the Inpatient Care Manager helps ensure safe, timely, and patient-centered care.
Description
Responsible for providing care and services of selected member populations across the continuum of illness. Ensures effective utilization and monitors health care resources. Manages the interdisciplinary team to achieve optimal clinical and resource outcomes. Works with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of the patient.
Principal Accountabilities:
Manage, coordinate, and facilitate patient care through comprehensive patient and family assessments and multidisciplinary collaboration.
Perform daily chart reviews, utilization reviews, and coordinate discharge planning activities, ensuring thorough and accurate clinical documentation in the electronic medical record.
Lead and participate in interdisciplinary patient care rounds, identifying and resolving quality of care issues, barriers to care progression, and discharge delays.
Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions.
Collaborate with the interdisciplinary healthcare team to manage and support patient care plans.
Perform variance tracking and outcome analysis to assess the effectiveness of clinical pathways and identify opportunities for improvement.
Serve as a representative of the department and participate in hospital and community outreach activities as appropriate.
Perform other related duties as assigned.
Qualifications
Education:
Required
Bachelor of Science in Nursing (BSN)
Current, valid New Jersey Registered Nurse license
Preferred
Minimum of three years of acute care experience strongly preferred
