Opportunity InformationSutter West Bay Medical Group (SWBMG) is seeking an experienced
Cardiology Nurse Practitioner (NP) to support our non-invasive cardiology program, with a focus on our high-volume
pre-transplant cardiovascular evaluation and longitudinal care for our
liver and renal transplant Programs of Excellence to support our
Advanced Organ Therapy Division at Van Ness Campus (VNC) / California Pacific Medical Center (CPMC).
This is a unique opportunity to practice at the intersection of
advanced cardiovascular medicine and solid-organ transplantation within a
quaternary, academically oriented health system. The role is embedded in a highly collaborative, multidisciplinary environment alongside transplant hepatology, nephrology, cardiology subspecialists, anesthesia, and surgery.
Clinical Focus• Provide comprehensive
cardiovascular assessment and optimization for liver and renal transplant candidates
• Lead and manage
pre-transplant cardiology clinics, including risk stratification and longitudinal follow-up
• Collaborate closely with:
- Liver and renal transplant teams
- Advanced heart failure and transplant cardiology
- Advanced cardiovascular imaging
- Interventional cardiology and electrophysiology
• Coordinate diagnostic testing, guideline-directed medical therapy, and peri-transplant cardiovascular planning
• Participate in multidisciplinary transplant selection conferences
• Support quality improvement, protocol development, and program growth within Advanced Organ Therapy

Qualifications• Graduate of an accredited
Nurse Practitioner program•
Board certified as a Nurse Practitioner
• Prior experience in
cardiology,
advanced heart failure, or
transplant medicine a plus• Comfort managing complex, high-acuity patients with multisystem disease
• Strong clinical judgment, communication skills, and commitment to patient-centered care
• Interest in working within a collaborative, team-based, academic community hospital environment
