The pay scale is $199.804.80 to $287,539.20

Position Summary
The Associate Vice President, Enterprise Care Management provides executive leadership for MultiCare's integrated care management continuum, including acute care case management, ambulatory care management, patient progression, care coordination, and transitions of care. This role is accountable for designing and advancing an enterprise-wide operating model that connects inpatient, ambulatory, post-acute, and value-based care strategies to improve outcomes, reduce avoidable utilization, support appropriate length of stay, and strengthen financial and operational performance.

This position leads system-level transformation of care management services across the enterprise, establishing standardized models, governance structures, performance metrics, workflows, and accountability mechanisms. The AVP partners closely with physician leadership, nursing, operations, finance, population health, payers, and community partners to advance patient access, quality, experience, regulatory compliance, and total cost-of-care objectives. The role is responsible for strategic planning, care model innovation, budget and resource stewardship, workforce planning, leadership development, and implementation of evidence-based practices that create consistent, high-performing care management services across MultiCare.

Responsibilities

  • Provide executive leadership for the enterprise care management continuum, including acute case management, ambulatory care management, patient progression, transitions of care, and care coordination.
  • Design and implement an integrated enterprise operating model for care management across inpatient, ambulatory, post-acute, and value-based care environments.
  • Establish system-wide standards, governance structures, workflows, policies, performance metrics, and accountability mechanisms for care management services.
  • Lead strategies to improve patient progression, reduce avoidable utilization, support appropriate length of stay, improve discharge effectiveness, and advance total cost-of-care performance.
  • Partner with physician, nursing, operational, finance, population health, and payer leaders to align care management strategies with quality, access, patient experience, and financial objectives.
  • Lead enterprise transformation initiatives that standardize care management practice, improve care coordination, and support scalable, evidence-based service delivery.
  • Oversee workforce planning, leadership development, succession planning, and performance expectations for care management leaders and teams.
  • Provide budget accountability, resource planning, productivity oversight, and business case development for care management programs and initiatives.
  • Leverage data, analytics, regulatory requirements, and outcomes reporting to identify opportunities, monitor performance, and drive continuous improvement.
  • Build and maintain collaborative relationships with internal leaders and external stakeholders, including providers, payers, community partners, post-acute providers, and regulatory agencies.


Requirements
  • Master's degree in Nursing, Healthcare Administration, Business Administration, Public Health, or related healthcare field required.
  • Active RN license or LICSW required. RN licensure strongly preferred when required by operational scope.
  • Ten years of progressive healthcare leadership experience required.
  • Five years of senior leadership experience required in one or more of the following: care management, case management, utilization management, patient progression, population health, ambulatory operations, clinical operations, or value-based care.
  • Demonstrated experience leading enterprise-wide clinical, operational, or care continuum transformation initiatives.
  • Demonstrated ability to lead across matrixed environments with physician, nursing, operational, finance, and external stakeholder partners.
  • Strong financial acumen, strategic planning capability, regulatory understanding, change leadership, and performance improvement expertise required.
  • Demonstrated ability to use data and analytics to improve quality, utilization, patient progression, access, and financial outcomes.
  • Exceptional communication, executive presence, stakeholder engagement, and leadership effectiveness required.
