Responsibilities
- Oversees daily clinical and field operations related to in-home care delivery of HRC patients for the assigned market, including direct supervision and in-home shadow visits with the acute care RN team.
- Provides daily clinical and supervisory support for the assigned shift, including weekend rotation.
- Provides information to leadership necessary to generate clinical reports.
- Generates relevant clinical and quality reports and provides/presents for market updates regularly.
- Develops ACRN assignment/schedule in conjunction with ACCM and partnering supervisor as needed for all in-home care delivery of clinical staff and network providers (PT, OT, etc.)
- Works closely with the ACCM, RCCs and HRC leader to facilitate communication and coordination between all members of the care team.
- Supervises ACRN clinical work and recovery course including daily clinical documentation and chart review and home shadowing visits.
- Guides ACRNs to promote effective performance and delivery of quality in-home care services through effective onboarding, education, mentoring and/or remediating clinical skills/processes/critical thinking,
- Assists ACRN team members in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing the care plan.
- Performs other duties as assigned.
Qualifications
- Associate of Science in nursing.
- Current, active RN state licensure.
- Two years of acute care nursing experience.
- One year of experience managing direct reports.
Preferred - Bachelor of Science in nursing.
- Three months of experience as an acute care RN providing in-home care.
