Qualifications

• Registered Nurse (RN) license with at least 3 years of healthcare experience, preferably in hospice or similar environments.
• Bachelor's degree in nursing strongly preferred.
• Leadership or management experience is highly valued.
• Valid driver's license is necessary.

Responsibilities

• Lead and manage an interdisciplinary team including nurses, aides, social workers, and chaplains.
• Ensure compliance with hospice regulations and standards.
• Facilitate effective communication between medical staff and the care team.
• Conduct interdisciplinary meetings to document patient needs and care plans.
• Create and refine patient care policies and emergency procedures.
• Oversee staff scheduling, performance reviews, and enhance workplace culture.
• Serve as the main point of contact for families, physicians, and other care providers.
• Analyze quality metrics and manage a patient caseload as necessary.
• Participate in after-hours and on-call duties for continuous care.
• Maximize resource utilization and encourage a collaborative work environment.
