  • Lead meetings to document patient requirements and care plans.
  • Develop and regularly update patient care policies and emergency protocols.
  • Manage employee schedules, conduct performance evaluations, and cultivate a positive workplace environment.
  • Serve as the main communication link between families, doctors, and other healthcare providers.
  • Analyze quality metrics while managing a patient caseload when necessary.
  • Be available for after-hours and on-call duties to guarantee continuity of care.
  • Optimize resource use while encouraging a collaborative work culture.

  • Bachelor's degree strongly preferred.
  • Leadership or management experience a major plus.
  • Valid driver's license (required)

  • Ensure adherence to hospice regulations and standards.
  • Facilitate communication between medical staff and the care team.
  • Lead interdisciplinary meetings, documenting patient needs and care plans.
  • Develop and update patient care policies and emergency procedures.
  • Manage staff scheduling, performance reviews, and foster a positive work environment.
  • Act as the central hub for communication with families, physicians, and other providers.
  • Analyze quality metrics and manage a caseload of patients as needed.
  • Participate in after-hours and on-call duties to ensure seamless care.
  • Maximize resource utilization and promote a collaborative work culture.
