Description
Coverage Territory: Bloomington, Streator, Lincoln, and surrounding areas
Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation
What You'll DoAs a
Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team - stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
- Delivering professional nursing care across the full nursing process within your scope of practice
- Conducting thorough patient and family assessments and contributing to individualized care plans
- Completing all visit types as needed to meet patient and family needs
- Administering medications and treatments per physician orders
- Identifying nursing diagnoses from health assessment data and responding accordingly
- Recording accurate, timely clinical notes including pain and symptom management outcomes
- Communicating changes in patient condition to physicians and obtaining orders as required
- Coordinating with community health partners to support seamless care transitions
- Ensuring equipment and necessary services are arranged and available for patients
- Maintaining and growing your clinical skills through continuing education and in-service participation

Requirements
What You Bring- Active RN license in the state of employment
- Graduate of an accredited school of Nursing.
- Clinical experience in hospice, home health, or a related setting preferred
- Flexibility and confidence working across varied patient assignments
- Strong assessment skills and sound clinical judgment under pressure
- Excellent documentation habits and communication skills
- Valid driver's license and willingness to travel locally
Salary Description
$78,000 - $85,000 a year
