Job Overview

The IPA RN Inpatient Case Manager will support daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facility (SNF) in-person as well as telephonically. This position will also provide case management to patients who are admitted to the hospital or SNF, and those patients who may need to be enrolled in ambulatory case management. Working as part of an interdisciplinary care team in collaboration with the Medical Management Team, the IPA case manager will be responsible for coordinating care, including supporting patient discharge planning (to home, SNF, Long-Term Care Facility, etc.), coordinating with IPA clinics, scheduling post-discharge visits with the IPA Clinical Care Team as well as with the IPA Primary Care Provider.

Minimum Requirements

  • Graduation from an accredited nursing program.

  • Current valid License as a Registered Nurse through the California Board of Registered Nursing.

  • Bachelor’s degree in social work, nursing, or another health or human services field with the appropriate licensure preferred.

  • Minimum of 1 year of experience in public health nursing, acute care, case management, and/or home health care is preferred.

  • Minimum of 1 year of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care is preferred.

  • Experience and familiarity with Utilization Management (UM) standard processes.

  • Experience with data analytics software preferred (e.g., Cozeva, Tableau).

  • Experience with physically or mentally impaired adults and/or the geriatric population, and working as part of a multi-disciplinary team.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

  • Bilingual: English/Spanish.

Compensation

Pay for this job starts at $104,000.00 annually
