Qualifications

Summary

Job Description:

Job Summary
Responsible for the entire process of sterile compounding; assuring quality assurance goals are met by
the organization and maintaining compliance with State and Federal laws and standards related to sterile
and hazardous compounding. Develops and performs competencies for staff members in the area of sterile
and hazardous medication compounding; maintain appropriate records related to sterile compounding and
monitor quality assurance program for sterile compounding Performs as a staff pharmacist within the sterile
compounding environment. Responsible for medication preparation and distribution; evaluating, inputting
and verification of new medication orders; assisting providers and other health care professionals by
serving as a medication information resource; and managing pharmacy inventory.

Minimum Education and Training

Bachelor's Degree from an accredited College of Pharmacy

Kentucky and/or Indiana Board of Pharmacy license in good standing as applicable to practice site

Two (2) years of hospital experience in sterile compounding (as a pharmacist or technician) required

Active Board Certification in Compounded Sterile Preparations Pharmacy preferred
