Full Job Description
Summary:
The purpose of the pediatric hematology nurse navigator is to:

  1. Help pediatric hematology patients navigate the healthcare system with a focus on inpatient to outpatient transitions
  2. Personalize the care our patients receive
  3. Provide an educational resource for patients and families
  4. Provide consistent contact and communication for pediatric hematology patients


The nurse navigator facilitates care of the patient and family across the many disciplines and professionals required for their care. The nurse navigator serves as the liaison for the patient and family addressing their concerns and questions and to coordinate patient care.

Responsibilities:

1. Communication:

  • Nurse Navigators will ensure that there is continuity of communication among the patient's medical team (both internal and external to UNC Health).


2. Nurse Navigator Continuing Education:

  • Serves as an educator on the healthcare team for hematology-specific education and disease management.

3. Patient Care Duties:

  • Interviews patients regarding their current health practices, identifies health problems, determines the degree of compliance and investigates barriers with adherence of the prescribed treatment plan, and documents this information in the patient's chart.
  • Analyzes data from patient interviews, chart reviews, and physical assessments to determine patient's nursing care needs including direct care and referral to other departments. Coordinates scheduling of appointments, labs, infusions, imaging, and consults. Interacts with referring physician's offices for new and ongoing patient care issues.
  • Obtains prior authorization and supports approval of new medications.
  • Assists with referrals, home health, and school forms.


4. Programmatic Leadership:

  • Functions as part of a comprehensive hematology team.
  • Provides cross coverage for other team members and assists with process development, implementation of new programs, and other initiatives.

Education Requirements:
• Bachelor's degree in Nursing (BSN) from a state-accredited school of professional nursing.
Licensure/Certification Requirements:
• Licensed to practice as a Registered Nurse in the state of North Carolina.
• Basic Life Support (BLS) for Healthcare Provider certification
Professional Experience Requirements:
• Two (2) years of nursing experience and one (1) year in the area of specialization.
