What You Will DoThis is an exceptional opportunity for a motivated advanced practice provider to join an employed position within a busy, well-respected practice, supported by a strong referral base and the full resources of a robust health system network.
- Join an established team of physicians and APPs within a highly collaborative, interdisciplinary palliative care team supervised by Dr. Colleen Brown
- Call structure: Evenings and weekend home call only (approximately 1 night/week and 1 weekend/month; low call volume averaging 0-3 pages/weeknight and 0-5 total pages/weekend)
- Practice setting: Inpatient (managing 5-8 patients per day based on experience and acuity)
- Access to Sunrise Clinical Manager (SCM) EMR system, supportive multidisciplinary care teams, and specialized clinical resources across Level 1 Trauma, Cardiovascular, Stroke, and Oncology programs
- Employed position within a nonprofit health system
- Board Certified or Board Eligible as a Nurse Practitioner (NP)
- Eligible for Indiana state medical licensure and DEA registration
- Perform high-level inpatient palliative care consults and clinical rounds, providing comprehensive pain, symptom management, and psychosocial support while managing complex goals of care for patients with new or existing serious illnesses.
- Drive multidisciplinary care coordination and discharge planning in partnership with community-based specialists, primary care providers, and hospice care teams to ensure a seamless continuum of care following evidence-based best practices.

What You Will NeedLicensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.
- Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Education:
- Master's degree of Nursing required.
