Facility: Mercy Hospital

Department/Specialty: Pharmacy Service

Schedule: Evening shift| 40hrs wk|Hours: 10hr shifts, 7:30am to 6pm or 1pm to 11:30pm|Every third weekend and every third holiday rotation.

How you'll make an impact in this role

Core Pharmacist Responsibilities: Safe and Effective Medication Management

Order Management: Review and validate all medication orders for clinical appropriateness. Screen for interactions, duplications, and contraindications. Resolve issues with prescribers and document interventions.

Preparation & Dispensing: Accurately compound specialized sterile/non-sterile medications (e.g., chemo, TPN) per regulatory standards. Ensure correct dispensing, labeling, and inventory control.

Clinical Expertise & Education: Provide expert pharmacotherapy information to providers. Offer comprehensive, patient-centered counseling on complex medications (action, administration, side effects, adherence).

Collaboration & Care Coordination: Participate in rounds and committees. Develop patient-specific medication plans. Perform medication reconciliation upon admission/discharge to prevent errors and ensure safe transitions of care.

Quality & Development: Supervise technicians, ensuring quality control and regulatory compliance. Mentor students/residents. Manage various operational areas (central, satellite, specialty services).

Patient Consultation: Conduct focused consultations for patients on high-risk/complex regimens (e.g., anticoagulation, insulin) to optimize adherence and therapeutic outcomes.

Licensure / Certification / Registration:

  • Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.


Education:
