Job Title: Home Health Registered Nurse (RN)
Location: Marlton, NJ (Field visits across Burlington, Camden, and Gloucester Counties)
Job Type: Full-time
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (40 hours/week)
Pay Range: $45.00 - $50.00+ / hour (commensurate with total years of nursing experience)
Key Responsibilities - Comprehensive Patient Care: Conduct thorough physical and psychosocial assessments, monitor vital signs, and administer direct nursing interventions in patient homes.
- Care Coordination: Collaborate closely with physicians, interdisciplinary healthcare teams, patients, and families to design, implement, and adjust individualized treatment plans.
- Patient & Family Education: Instruct patients and caregivers on disease management, safe medication administration, home safety protocols, and general wellness.
- Supervision & Quality: Directly supervise Home Health Aides (HHAs) in the field and participate in continuous quality improvement initiatives.
- Documentation: Complete clinical documentation accurately and promptly using point-of-service technology in accordance with agency standards.
Minimum Qualifications - Education & Credentials: Active, unencumbered New Jersey RN License.
- Clinical Experience: 3 to 4+ years of hands-on nursing experience in MedSurg, Intensive Care (ICU), Critical Care (CCU), or acute hospital settings.
- Core Competencies: Proficiency in manual vital sign collection (blood pressure, heart rate, respiration rate) and essential bedside clinical skills.
- Travel: Reliable transportation to complete field visits throughout Burlington, Camden, and Gloucester counties (mileage reimbursed at the standard government rate).
Preferred Qualifications - Background in Certified Home Health with hands-on OASIS assessment experience.
- Prior experience as a Licensed Practical Nurse (LPN) prior to RN licensure.
