Position Profile

Right now we're looking for a Clinical Coordinator, Home Dialysis to join our team within the Nephrology Program.

Using a collaborative and consultative approach, the Independent Dialysis Clinical Coordinator, will assist in providing program/service leadership to create an environment for innovation and excellence in patient focused care. This position is accountable to the Manager of the Kidney Care Clinic, Transition Care Unit and Home Dialysis Program. This position will take the lead in a comprehensive approach with all patients starting dialysis by liaising with the patient/family, Nephrologist and Interdisciplinary team with the purpose of providing assessments, education, treatment option shared decision making along with support and follow up while encouraging independent dialysis modalities. The Home Dialysis Clinical Coordinator is also accountable to the Ontario Renal Network through the Regional Director, and responsible for ORN Independent Dialysis assessment and data collection for the program.

Responsibilities:
  • Providing high quality, effective, efficient and safe patient services through human, environmental and financial resources management.
  • Assisting the Nephrology Program Managers, Resource Nurse/Clinicians/TCU Staff with the day-to-day processes that enable the delivery of high quality services to meet patient needs and expectation.
  • Collaborating with the Ontario Renal Network to lead and evaluate implemented quality improvement initiatives and reporting on their progress.
  • Organizing and coordinating the activities of related clinical areas as needed, including nursing and ancillary staff activities to meet patient needs and in collaboration with all members of the Dialysis department.
  • Collaborating with Kidney Care Clinic, Transition Care Unit, Home Dialysis/In-centre Resource Nurses, Allied Staff Workers and Physicians to coordinate transfer and care of pre dialysis, New Start Unit, Transition Care Unit, home dialysis patients and potential home dialysis patients within the Humber River Nephrology Program.
  • Collaborating patient assessment, preparation, post-insertion interventions for dialysis body access with Body Access Coordinator, and Kidney Care Clinic and Home Dialysis Nurse Clinicians.
  • Collaborating with the Home Support Nurse and Transition Care Unit nurses in the daily and future planning of home dialysis and TCU patient training flow and assisting in staff assignments when necessary.
  • Participating in research projects, data/statistics collection and reporting to appropriate stakeholders.
  • Assisting the CPL with the development of staff to achieve advanced clinical practice standards for the Kidney Care Clinic, New Start Unit, Transition Care Unit and Home Dialysis Program.
  • Implementing internal linkages and coordinating services in response to the needs of the patients.
  • Providing leadership and consultation to staff in the development, implementation and evaluation of related projects and initiatives for Independent Dialysis Program (Home Dialysis).
  • Demonstrating an empathetic, resourceful and direct approach to successfully assist patients through their renal journey and in their adjustment to starting dialysis.
  • Acting as an expert resource to the clinical members of the Dialysis Program.
  • Facilitating and directing patient flow in, out and throughout the Nephrology Program.
  • Assigning patients to primary nurses in the Home and Transition Care Units.

Requirement:
  • Registered and in good standing with the College of Nurses of Ontario
  • BLS Provider certified
  • CNeph Certification Required
  • Completion of a Baccalaureate Degree in Nursing required
  • Minimum of 5 years previous clinical experience in Nephrology nursing, with knowledge of all components of independent dialysis with emphasis in home dialysis therapies.
  • Experience in all aspects of body access including peritoneal dialysis catheters and vascular access for hemodialysis required
  • Experience with ORN data collection (ID trackers/VE assessment) preferred.
  • Previous experience in pre-dialysis transplantation, home hemodialysis and peritoneal dialysis required
  • Demonstrated computer skills using Microsoft office (Word, Excel, Power Point, Outlook, Access) and Meditech
  • Demonstrated ability in adult teaching, knowledge of learning principles and assessing readiness to learn
  • Demonstrated leadership skills in program planning and development
  • Experience in developing/implementing patient algorithms and care paths
  • Demonstrated excellent interpersonal and communication skills with the ability to work with a multidisciplinary team including other health team members, patient/family
  • Experience in developing and participating within Continuous Quality Indicator programs
  • Demonstrated excellent patient assessment skills
  • Demonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customers
  • Strong communication and interpersonal skills; listens for clarity and meaning, and to speak in an honest and straightforward manner to ensure mutual understanding in a conversation

  • Knowledge of second language (Spanish or Italian) preferred.
  • Valid driver's license required and access to reliable transportation for travel between all HRH sites and participate in home visits if needed
  • Excellent attendance and discipline free record required.
