Clinical Social Worker (LCSW)

Full-Time | Monday - Friday | Onsite | Winchester, VA

$83,000 - 96,000 per year. The actual hiring rate may vary based on several factors, including your previous work experience, educational background, the responsibilities of the position, and demonstrated performance. Responsibilities

  • Provide therapeutic counseling, crisis intervention, and emotional support to patients and families.
  • Conduct psychosocial assessments and develop individualized support plans.
  • Participate in distress screening follow-up, survivorship care, and advance care planning discussions.
  • Connect patients with financial, community, and supportive resources.
  • Serve as a patient advocate and help navigate healthcare and community systems.
  • Facilitate support groups and collaborate with community organizations.
  • Partner with providers and clinical staff to address psychosocial barriers to care.
  • Maintain timely and accurate documentation in the electronic medical record.

Qualifications
