Clinical Lead - Multi-Site Outpatient Assisted Living

Independence Rehab is seeking a compassionate Clinical Lead who finds purpose in helping older adults maintain and regain mobility, confidence, and independence-while leading therapy teams that deliver meaningful outcomes every day.

This position provides clinical services and operational leadership for Independence Rehab's outpatient therapy programs across four Portland-area assisted living communities:

• Rose Villa Senior Living
• Regency Park
• Deerfield Village
• Royalton Place

This is a hands-on leadership position that combines patient care with multi-site clinical oversight. You'll help develop clinicians, support consistent clinical and operational standards across locations, and build strong relationships with the leadership teams at each community.

What a Day Looks Like

As Clinical Lead, your work is relationship-driven, collaborative, and highly impactful.

You'll maintain an active patient caseload while providing leadership and oversight to therapy services across all four Portland-area locations.

Your responsibilities include:
• Providing clinical and operational oversight for therapy programs at Rose Villa, Regency Park, Deerfield Park, and Royalton Place
• Serving as the primary therapy leadership resource across the four communities
• Overseeing full-time, part-time, and PRN therapy staff
• Maintaining a regular clinical caseload, primarily at Rose Villa
• Visiting and supporting each satellite location based on program and staffing needs
• Building strong relationships with facility leadership and administration at each community
• Attending multidisciplinary and facility management meetings as appropriate
• Interviewing and hiring therapy staff in partnership with Recruiting
• Creating and managing therapy team schedules and helping coordinate staffing across locations
• Monitoring program performance, revenue, and expenses to support sustainable operations
• Serving as a key resource for patients, families, caregivers, and facility partners
• Monitoring documentation quality and ensuring compliance with regulatory and company standards
• Training, mentoring, and developing clinicians
• Helping establish consistent clinical practices and expectations across all four locations
• Identifying opportunities to strengthen therapy utilization, patient outcomes, and program growth

Education & Experience
• Current Oregon Physical Therapist Assistant license in good standing
• Minimum one year of geriatric clinical experience; SNF, assisted living, or senior living experience strongly preferred
• Prior DOR, Clinical Lead, or therapy management experience preferred but not required
• Ability and willingness to provide leadership across multiple Portland-area locations
• Strong understanding of therapy documentation, regulatory compliance, and reimbursement requirements
• Strong organizational skills with the ability to prioritize responsibilities across multiple programs
• Driven, teachable, detail-oriented, and dependable
• Comfortable leading, motivating, mentoring, and developing others
• Strong relationship-building and communication skills
