Position Summary

The Clinical Coordinator provides operational and clinical coordination for the Spinal Cord Program within the Lyndhurst Centre Outpatient Clinic. The role supports the delivery of interdisciplinary outpatient services by coordinating referrals, triaging patients according to program criteria, managing waitlists, facilitating communication between patients and clinical teams, and supporting efficient clinic operations.

The successful candidate will work closely with physicians, physiatrists, nurses, occupational therapists, physiotherapists, rehabilitation assistants, administrative teams, and external healthcare providers to ensure patients are appropriately assessed, prioritized, scheduled, and connected with the most appropriate rehabilitation services.

The role also supports workflow improvement, service planning, data tracking, and reporting, identifying operational trends and opportunities to improve patient access, referral management, and clinic efficiency.

Duties
  • Coordinate day-to-day operations for the Spinal Cord Program outpatient clinic.
  • Receive, review, and coordinate referrals for outpatient rehabilitation services.
  • Triage referrals according to clinical program criteria, patient needs, service availability, and urgency.
  • Coordinate patient access across interdisciplinary services including PT, OT, supervised gym, and specialized clinics.
  • Manage and monitor waitlists, referral volumes, appointment availability, and patient flow.
  • Communicate with patients, families, referring physicians, hospitals, community providers, and internal clinical teams.
  • Facilitate communication between interdisciplinary team members to support timely patient care.
  • Identify referrals requiring physician and facilitate appropriate escalation.
  • Coordinate transitions between inpatient, outpatient, acute-care, and community services when appropriate.
  • Monitor operational issues affecting access, including staffing capacity, referral volumes, appointment availability, and changes in inpatient/outpatient demand.
  • Develop and maintain tracking tools, reports, and dashboards to monitor clinic activity and service demand.
  • Analyze referral and appointment data to identify trends, capacity issues, and opportunities for workflow improvement.
  • Support implementation of process improvements designed to improve patient access and clinic efficiency.
  • Participate in interdisciplinary meetings, program planning, and operational discussions.
  • Develop and maintain standardized processes, communication templates, and operational resources.
  • Provide education and clarification to referring providers regarding program eligibility, referral requirements, and available services.
  • Ensure documentation and communication are completed accurately and in accordance with UHN policies and clinical standards.
  • Collaborate with leadership on service planning, performance monitoring, and operational priorities.


Qualifications
  • Completion of a bachelors degree in Science, Nursing, or recognized equivalent required. Masters degree preferred, but not required
  • Completion of a Basic Cardiac Life Support (B.C.L.S.) program, Level II Rescuer certification required
  • Minimum 2 years clinical background is required (nursing or health professions)
  • Experience working in an advanced clinical practice role
  • Certificate of competency (licensure) from the College of Nurse of Ontario (CNO) required
