Position Summary

The Clinical Coordinator is an advanced healthcare professional responsible for the clinical coordination and facilitation of complex patient care activities across multiple specialties and services. Leveraging expert clinical knowledge, critical thinking, and professional judgment, the Clinical Coordinator serves as a key liaison between patients, families, physicians, multidisciplinary care teams, external healthcare providers, and funding organizations. The role is accountable for navigating highly complex clinical situations, facilitating access to specialized care, supporting treatment planning and service delivery, and ensuring a seamless patient experience. Through collaborative practice, relationship management, and effective resource stewardship, the Clinical Coordinator contributes to excellence in patient care, operational effectiveness, and program performance.

Duties:
  • Provide clinical leadership in the coordination and management of complex patient cases, ensuring timely progression through consultation, assessment, treatment planning, and follow-up care.
  • Utilize advanced clinical knowledge and professional judgment to assess patient needs, synthesize clinical information, identify potential risks or barriers to care, and support informed decision-making throughout the care continuum.
  • Act as the central point of coordination among patients, families, physicians, multidisciplinary teams, referring providers, and external stakeholders to facilitate seamless and integrated care delivery.
  • Collaborate with physicians and healthcare teams to support clinical review processes, treatment recommendations, care planning activities, and coordination of specialized services.
  • Facilitate access to consultations, diagnostic investigations, procedures, treatments, admissions, and follow-up services across multiple programs and healthcare environments.
  • Develop and maintain strong working relationships with healthcare providers, healthcare institutions, insurers, embassies, government agencies, case managers, and other funding organizations to support continuity of care and service delivery.
  • Navigate complex funding and authorization processes by coordinating clinical documentation requirements and acting as a liaison between clinical teams and third-party payors.
  • Demonstrate financial stewardship through the interpretation of treatment plans, review of proposed services, monitoring of care authorizations, and identification of potential variances that may impact approved funding or resource utilization.
  • Anticipate, assess, and resolve clinical, operational, and system-level challenges that may affect access to care, patient outcomes, service delivery, or the overall patient experience.
  • Serve as a clinical resource and trusted advisor to patients, families, and stakeholders by providing education, guidance, and support regarding care pathways, treatment options, and healthcare services.
  • Ensure accurate and comprehensive documentation in accordance with professional standards, privacy legislation, accreditation requirements, and organizational policies.
  • Contribute to quality improvement initiatives, program development, process redesign, and other strategic activities aimed at enhancing patient outcomes, service excellence, and operational performance.
  • Participate in organizational and cross-functional initiatives that promote innovation, collaboration, and continuous improvement.


Qualifications
  • At least 3 years of clinical practice or clinical coordination experience
  • Regulated healthcare professional (DC, PT, RD, RT, RP, RN, OT, SLP)
  • Experience incorporating equity, diversity, inclusion, and accessibility principles into daily practice, fostering respectful, inclusive, and culturally responsive interactions with patients, families, colleagues, and external stakeholders.
  • Applicants must be members in good standing, or eligible for registration, with a regulated health profession recognized under Ontario's Regulated Health Professions Act (RHPA).
  • Sound understanding of privacy, confidentiality, and consent requirements, including applicable provincial and federal legislation (e.g., PHIPA and PIPEDA), with the ability to safeguard personal and personal health information in a complex healthcare environment.
  • Practice in accordance with applicable professional, ethical, regulatory, and occupational health and safety standards.
  • Understand
  • Proficient use of computer, software, and peripheral equipment, i.e., Windows, MS Office, MS Mail/Outlook, MS Project, MS Copilot.

Professional Affiliations/Memberships:
  • Membership in relevant Ontario College of Health-Related Discipline required
  • Membership in relevant professional agencies and associations preferred
