Overview

About the Role

The primary purpose of the Clinical Care Coordinator position is to serve as the coordinator and practitioner of the clinic's healthcare team(s) within the Women's and Children's Service Lines , utilizing specialized knowledge, skills, and competencies in behavioral sciences, education counseling, and/or patient management. Develops patient programs and course curricula based on needs assessments; plans and coordinates the delivery of comprehensive services to patients in collaboration with all professional components of the clinic and in accordance with accepted standards of care and department protocols..

Responsibilities

  • Provides educational services and programs for patients, families, and staff within the framework of the department.
  • Meets the needs of select patients through a care management approach.
  • Serves as an active member of a consortium of providers of care within the clinic to standardize the management of the patients.
  • Participates in the development of staff associated with the management of the disease.
  • Maintains a high standard of professional responsibility and performance.
  • Participates in research and performance improvement activities


Qualifications

What You'll Bring
  • Bachelor's degree in nursing (BSN) or a related health care field. A master's degree in nursing, Health Care Administration, Public Health, or a related field is preferred.
  • A minimum of three (3) years of full-time experience in a hospital-based outpatient practice is preferred.
  • Effective team leadership and interpersonal skills are required.
  • National Certification in Obstetrics or Pediatric Nursing preferred.
  • Must hold CPR certification.
  • Be knowledgeable in Managed Care and Healthcare Industry trends.
  • Two (2) years' experience in Maternal-Child;OB/GYN specialties preferred to include but not limited to, Pediatrics, Reproductive Choice.
  • Additional clinical experience can substitute for the degree.

Compensation

In compliance with New Jersey's Pay Transparency Act, the salary range for this position is PS/22S: $ 93,912.00 - $ 113,588.00 - $ 135,577.00. University Hospital considers multiple factors when determining compensation, including (but not limited to) the scope and responsibilities of the position, the candidate's relevant work experience, education and training, key skills, internal equity, market data, and organizational needs. The expected salary range for this position is in accordance with the New Jersey Pay Transparency Law; final compensation will be commensurate with experience and qualifications.
