Position Introduction: As the Director of Community-Based Case Management Programs, you will serve as a key member of the leadership team, partnering closely with the Executive Director to shape the strategic direction of the department while ensuring the successful day-to-day operation of multiple revenue-generating clinical programs. This position offers the unique opportunity to balance strategic planning with hands-on operational leadership, helping to advance programs that directly improve access to quality healthcare services within the communities we serve.

The successful candidate demonstrates sound judgment, excellent communication skills, and the ability to make informed decisions in a fast-paced environment while maintaining a strong commitment to integrity, collaboration, and continuous improvement. Candidates must possess a bachelor's degree in Nursing, Public Health Administration, or another health-related field (or an approved education and experience equivalent) and have at least two years of management experience in a healthcare setting. A master's degree in Social Work, Nursing, Public Health, Healthcare Administration, or a related health field is strongly preferred. Registered Nurses must maintain an active Oklahoma RN license, and certain programs may require additional certifications based on operational needs.

Required Education and Experience: - Bachelor's Degree in Nursing, Public Health Administration, or other health-related field.
- 24 months experience in a management role in a health care environment
Equivalency/Substitution: Associate's Degree in Nursing or other health-related field and 24 months experience in health care or related experience in lieu of the Bachelor's Degree in Nursing. All other degree fields experience or a combination of education & related experience can be considered in lieu of degree. A one-to-one ratio is used to determine the number of years of experience required in place of a degree.
Required Certifications and Licenses: - Certain departments may require additional certifications, licenses, or credentials based on the specific scope of work and applicable regulatory requirements.
- Nurses must possess an active Oklahoma Registered Nurse license
- Basic Life Support (BLS) certification
- Cardiopulmonary Resuscitation (CPR) certification.
Department Preferences: - Masters degree in social work, nursing, public health, or health related field preferred
- Experience supervising multidisciplinary teams and developing staff and managing community-based case management programs or similar population health services.
- Experience supporting or leading strategic planning and program development initiatives.
- Experience in audit readiness, Standards of Care, or regulatory reviews for programmatic compliance.
- State of Oklahoma Registered Nurse, Licensed Clinical Social Worker, National Certified Case Manager
- One year of paid professional experience in direct service provision, care planning, and has provided services to the aging and/or disabled population.
- Experience in a home-like setting.
