POSITION SUMMARYIn alignment with the mission and strategic direction of Neighborhood House, the Associate Director of Community Health plays a critical role in strengthening the infrastructure, systems, and day-to-day operations that support our expanding Community Health programs, including the Center 55+, Food Security, Housing, and HRSN initiatives.
The purpose of the Associate Director of Community Health is to ensure that Community Health programs are impactful, sustainable, scalable, and well-supported. By establishing strong systems and infrastructure, the Associate Director of Community Health empowers program teams to focus fully on delivering effective community services.
This role works closely with the Director of Community Engagement and Community Health to translate strategic priorities into clear, functional systems and workflows that improve program delivery, staff efficiency, community engagement, and participant experience. This role directly supervises the Community Health Program Managers and collaborates with other internal and external stakeholders to implement and maintain effective program and operational systems and frameworks.
JOB DUTIES & RESPONSIBILITIESLeadership and Supervision- Directly supervises Community Health Program Managers, providing ongoing coaching, evaluation, and accountability.
- Holds monthly 1:1s with direct reports to review program performance, compliance, staff management, and strategic initiatives.
- Hold monthly team meetings with program management and leadership teams.
- Oversees the professional development and performance management of direct reports and supports them in effectively supervising and developing their staff.
- Ensures effective communication, coordination, and alignment between program sites and management functions.
- Promotes a positive, inclusive, and mission-driven organizational culture focused on accountability, transparency, collaboration, and excellence.
- Lead cross-functional initiatives that improve coordination, integration, and operational consistency across departments and programs.
- Collaborate with organizational leaders to identify opportunities for innovation, shared learning, and continuous improvement.
- Support organizational planning efforts and contribute to strategic decision-making related to community health services and operations.
- Represent the department in organizational committees, leadership meetings, and external partnerships as appropriate
Systems Design & Implementation- Partner with the Director of Community Engagement and Community Health to identify operational challenges, opportunities for increased efficiency, effectiveness, and program growth.
- Lead the design and implementation of systems, workflows, and resources and tools that improve consistency, scalability, and effectiveness across Community Health programs
- Translate strategic initiatives into clear, actionable processes for Program Managers and frontline staff
- Develop, create and maintain documentation (SOPs, process maps) to support system adoption and continuity
- Design internal and external guides and resources
- Lead organizational change management efforts related to new systems, technologies, and operational practices, including planning, training, implementation, and evaluation
Program Operations & Infrastructure- Provide oversight ofday-to-day operational alignment across Community Health programs to ensuresmooth and consistent execution
- Ensure seamless collaboration with the VolunteerCoordinator to develop and implement volunteer opportunities that advance Community Health programs.
Data, Reporting & Continuous Improvement
- Ensure the development and maintenance of systems for tracking program outputs and outcomes
- Supports Director of Community Engagement & Community Health in training program teams to help them enter accurate and consistent data collection
- With partnership from the Director of Community Engagement and Community Health, work with qualitative data to identify trends, gaps, and opportunities for improvement
- Provide regular updates and insights to the Director of Community Engagement & Community Health
- Assess community needs, organizational capacity, and funding opportunities to guide program growth and strategic priorities
- Develop and strengthen partnerships with healthcare systems, housing agencies, public agencies, nonprofits, and community-based organizations
- Ensure programs maintain high standards of quality, compliance, equity, responsiveness, and measurable impact
- Support data-informed decision-making and evaluation processes to improve organizational effectiveness and community outcomes
- Identify and pursue new funding opportunities, partnerships, and strategic initiatives that expand organizational impact

UPERVISORY RESPONSIBILITIESThis position directly supervises the Volunteer Coordinator AmeriCorps Vista and the Community Health Program Managers
- May supervise contracted, consultation, and community health support roles, as applicable

REQUIRED EDUCATION AND EXPERIENCE- Bachelor's degree in communications, nonprofit management, public administration, community development, social work or a related field
- A minimum of five (5) of progressively responsible leadership experience in program operations, systems design, nonprofit management, or a related field
- A minimum of three (3) years direct supervisory experience.
- Demonstrated experience building or improving systems, workflows, or operational processes
- Strong project management skills, with the ability to move initiatives from concept to implementation
- Experience working cross-functionally and supporting multiple programs or teams
- Experience managing or coordinating volunteer programs strongly preferred
- Excellent communication, leadership, relationship-building, and problem-solving skills.
- Ability to balance big-picture thinking with attention to detail
- Proficiency with data tracking tools and systems (e.g., CRM platforms, spreadsheets, project management tools)
REQUIRED CERTIFICATES, LICENSES, CREDENTIALS AND/OR REGISTRATIONS- This position requires regular travel between multiple sites. You will need a valid driver's license, insurance, and a personal vehicle or other reliable transportation.

Salary $77,000 - 83,800
